3. Provide copies of Permanent Record Cards, transcripts, and grades upon receipt of bona fide requests by       students, former students, schools, or public agencies

Q. How do I request a transcript of my high school course work?
A. Within five years of attendance, call the school most recently attended. After five years, call (909) 397- 4800 x3488. Request that a release form be mailed to you. When the signed release form is received back, the records you requested will be mailed.

Q. What assurance is there that my records will not be released to an unauthorized party?
A. A release form must be signed by the student before any school records can be released to the student, a school, and employer, or any other party.

Q. How long are student records kept by Pomona Unified School District?
A. Mandated student records are kept permanently.
